The Women’s Heart Health Program at St. Pat’s is a risk assessment, diagnosis and treatment program to help women identify risks for cardiovascular disease, to reduce those risks, to obtain diagnostic information and to receive treatment if necessary.
Components include:
- Initial Assessment
- Appointment with Ms. Sybrant, PA-C, or Dr. Matt Maxwell, to discuss results and make a plan for risk reduction
- Referral as needed for diagnostic testing or treatment
- Monthly phone calls from IHI staff to answer questions, check in on progress
- At six months, follow-up meeting with your medical provider
- Quarterly optional events that are educational, inspirational and fun
- At one year, final meeting with your medical provider. Certificate of participation and Women’s Heart Health Program gift
